To assist the team, we are seeking for a Healthcare Administrator. This person should ideally have prior healthcare experience and will have the opportunity to grow and advance.

Qualifications:

• Oversee the coordination of referrals to the agency from both within and outside the organization.
• Oversee all elements of patient care coordination.
• Examine whether the patients’ needs are being assessed on a regular basis.
• To assist them in meeting their recruitment and retention goals, clinical partners are coached, mentored, and precepted.
• Before approving the admission, look at the agency’s capacity to meet the patients’ expected clinical needs.
• Make sure the patient and partner assignments are proper.
• Examine patient referrals to determine whether or not they are suitable for admission.
• Participate in any obligatory center/agency surveys and reports (Licensure / JCAHO).
• Collaborate with other key company partners to ensure the agency’s clinical and financial success.
• Assure that the patient’s personalized POC is developed, implemented, and updated.

Skills:

• A valid CPR certificate is required.
• At least two (2) years of recent experience as a registered nurse.
• A graduate of an accredited/approved RN programme or a nursing degree from an accredited institution or university is required.
• You must have a current, unrestricted Registered Nurse (RN) licence in the state where you work.
• It is necessary to have a year of experience in home health care.
